Items to bring with you to your appointment:
1. Please provide the last four consecutive pay stubs for all employed household members. If pay stubs are not available, submit the contact information for your employer so that employment can be verified.
2. If any household member receives Social Security, SSI, or VA benefits, a copy of the most recent award letter for the current or upcoming year must be provided.
3. Additionally, please submit photo identification for all adult household members, Social Security cards or ITINs for all individuals who have one, and birth certificates for all minors residing in the household.
